Hospitals and Homelessness: Reducing ED Visits and Addressing Social Determinants of Care

According to the National Academy of Medicine, social determinants of health (SDOH) account for 80-90 percent of a patient's health outcomes. Knowing this, Providence Health developed a program designed to address the non-medical contributors to a patient's health, including environmental and socioeconomic factors. Becky Wilkinson, MSW, CWSA, and head of these efforts shares how Providence was able to use case management to get homeless populations off the streets, out of hospitals, and into homes—achieving a 41 percent reduction in ED utilization for patients with social determinants of health.

In this one hour webinar, Wilkinson will share:
